Adjustabilní intragastrické balóny v bariatrii – větší zastoupení respondentů ve srovnání s užitím neadjustabilních intragastrických balónů

Úvod:
Účinnost intragastrických balónů (IGB) a trvání jejich efektu může být rozdílné a nepředvídatelné. Adjustabilní intragastrické balóny (AIGB) firmy Spatz byly vyvinuté k prodloužení implantace na jeden rok, možnosti snížení objemu balónu pro intoleranci či zvýšení objemu balónu pro jeho nedostatečný efekt. Přínosnost/účinnost a míra odpovědi při použití Spatz3 AIGB byla subjektem tohoto výzkumu.

Metody:
Retrospektivně jsme hodnotili bez vyloučení výsledky 227 po sobě jdoucích pacientů s implantovaným Spatz3 AIGB ze tří center. Průměrné BMI bylo 35,9; průměrná váha 99,6 kg; průměrná nadhmotnost (EBWL) 30,6; průměrný objem balónu 464 ml (400–500 ml). Pacientům byla nabídnuta úprava objemu balónu: snížení objemu pro intoleranci a zvýšení objemu pro zpomalení váhového úbytku.

Výsledky:
Implantace byla provedená u 227 pacientů (průměr 10,3 měsíců) s dosažením průměrné redukce hmotnosti o 15,8 kg; průměrné redukci celkové hmotnosti (TBWL) 15,7 % a redukci nadhmotnosti (EBWL) 57,6 %. Odpověď na terapii (> 25 % EBWL) byla dosažena u 83,3 % pacientů. Snížení objemu balónu bylo provedeno u 15 pacientů (průměrně za 2,3 týdne; v průměru o –120 ml) a dovolilo pokračovat v terapii AIGB u 12 z 15 pacientů (80 %) s průměrnou dobou 8,8 měsíců. Navýšení objemu balónu u 107 pacientů (průměrně za 4,7 měsíců; v průměru o +284 ml) dovolilo dosáhnout přídatné redukci váhy v průměru o 8,5 kg. Skupina pacientů s navýšením objemu měla v době extrakce balónu průměrnou redukci hmotnosti o 17,8 kg; 17,2 %TBWL a 59,2 % EBWL. Byly zaznamenány dvě žaludeční ulcerace, jedna v důsledku abúzu NSAID a jedna se zhojila po snížení objemu.

Závěry:
V této retrospektivní práci zaměřené na 227 po sobě jdoucích pacientů se zavedeným Spatz3 AIGB bylo provedeno navýšení objemu balónu s dosažením průměrné 8,5 kg přídatné redukci hmotnosti u pacientů s oploštěním váhového úbytku a snížení objemu balónu se zmírněním časné intolerance u 80 % pacientů. Tyto dvě možnosti úpravy objemu by mohly být nástrojem k dosažení míry odpovědi 83.3 %.
